- If you want it, it’s your responsibility to request it
- You will probably have to pay a reasonable fee for it
- You can ask the doctor to explain it to you
- You can ask the doctor to make changes and/or re-write illegible items
- What you say to your doctor impacts the quality of the information in your record
- You can (and should) keep your record electronically
- Your medical record can’t be shared with others without your consent
- It’s your responsibility to make sure new doctors get copies of your old records
- Your medical record impacts your family
- ....and many people you will never meet....
