  2. You will probably have to pay a reasonable fee for it
  3. You can ask the doctor to explain it to you
  4. You can ask the doctor to make changes and/or re-write illegible items
  5. What you say to your doctor impacts the quality of the information in your record
  6. You can (and should) keep your record electronically
  7. Your medical record can’t be shared with others without your consent
  8.  It’s your responsibility to make sure new doctors get copies of your old records
